Example of Nobelguide dental implant planning

NobelGuide is a solution for the complete treatment and diagnostics, as well planning and placement of dental implants. The use of NOBELGUIDE™ is beneficial for both us and our patients.

We are able to plan ahead depending on the prosthetic needs and the current bone condition. Information about both prosthesis and data obtained from the 3D X-ray provides us with everything that is needed to choose the ideal position of an implant. Moreover, biomechanical, functional and aesthetic requirements as well as quantity and quality of the bone at the selected location are fully integrated in the decision making process.

NOBELGUIDE™ offers the option of surgery that is minimally invasive, secure and foreseeable. It also enables immediate functioning and pre-manufacturing of the prosthesis, thus allowing us to offer our patients the Sorriso in a Day ™ and Dentes em uma hora™ services.

As soon as the planning is completed and approved, the corresponding surgical model is designed. The model encompasses all aspects of the necessary planning, i. e. it is customized according to location, type and size of the implants planned. The surgical model assures that the surgery is minimally invasive, secure and foreseeable. The model can be used in all cases – tooth, several teeth or a full mouth / jaw replacement.

Workflow

Clinical diagnostics including configuration of a tooth

The doctor performs clinical tests and obtains permanent dental impressions for the study models in order to evaluate and define the tooth configuration that is ideal for the final restoration. A dental prosthetics technician produces the master model based on the permanent dental impression and designs the configuration of the tooth.

Fabrication of radiography guide

The configuration is judged upon clinical optimization and evaluation, resulting in a radiographic guide. Radiopaque markers used by the software to align the patient as well as isolated radiographic verification guide (double verification protocol) are added. In several cases, inspection windows are created to verify the proper insertion of the radiographic guide and the surgical model.

A bite index is produced to ensure the correct positioning of radiographic guide in the patient’s mouth during verification.

Digitalisation and calibration

Using the CT (CB) scanner the first scan is taken with the patient wearing the radiographic guide and the bite index. During the second examination, only the radiographic guide is needed. Just as the double verification protocol, the anatomy of the patient and the radiographic guide are also digitalised in the way that both anatomical data and prosthetic targets can be visualized. A reference curve of the object that is being calibrated automatically adjusts the radiographic guide settings to the surgical model.

Treatment diagnostics and planning, including team collaboration

NobelClinician Software enables the virtual diagnostics and treatment planning. Various working modes and functions allow the user to identify and note important anatomical structures. Moreover, it offers a virtual implant planning. The surgical model may be ordered to transfer a virtual plan to the clinical setting. NobelConnect enables the collaboration between the partners involved in the treatment via the secure platform of data sharing on the Internet.

Prefabrication of a prosthetic solution (optional)

Prosthetic dentistry technicians are able to prefabricate provisional tools in the laboratory using the surgical model, as well as to fabricate the final model based on the implant prior to the surgery.

Guided surgery

The surgical model, including the guide sleeves, is designed individually for each patient by means of Nobel Biocare using the centralised production method, and is delivered to the doctor. The surgical model is positioned using the surgery index, and the anchor pin(s) are placed to secure the model if needed. Drill guides are available in different diameters to lead the guided practice using a surgical model. Guided implant mounts are used to attach the models using a surgical model.
